Important instructions to follow after your bone graft for proper healing and recovery.

What to Expect

Congratulations on taking this important step toward rebuilding your jawbone! A bone graft is a foundation for future dental work, and proper healing is essential for long-term success. During the healing process, your body will gradually integrate the graft material with your existing bone, creating a strong, stable base for dental implants or other treatments.

It's completely normal to experience some discomfort, swelling, and minor bleeding for the first few days after your procedure. Most patients find that symptoms peak around day 2-3 and then begin to improve steadily. The initial healing phase typically takes 1-2 weeks, but complete bone integration can take 3-6 months depending on the size and location of your graft.

Remember, every patient heals at their own pace. Following these post-operative instructions carefully will help ensure the best possible outcome and minimize complications. Dr. Olaes and our team are here to support you through every step of your recovery.

Immediately After Your Procedure

For the first 24 hours, these steps are critical:

  • Keep the gauze pad in place for 30-45 minutes after leaving the office, applying gentle, constant pressure
  • If bleeding continues, place a fresh gauze pad over the area and bite down firmly for another 30 minutes
  • Do not remove or disturb the surgical site with your tongue, fingers, or any objects
  • Take prescribed medications as directed before the numbness wears off
  • Apply ice packs to the outside of your face for 15 minutes on, 15 minutes off during waking hours
  • Rest with your head elevated on 2-3 pillows when lying down
  • Avoid rinsing, spitting, or using straws for the first 24 hours
  • Do not smoke or use tobacco products (this can cause graft failure)

Pain Management

Managing discomfort is an important part of your recovery process. Here's how to stay comfortable:

Ice Therapy: Apply ice packs to the outside of your face for the first 24-48 hours. Use 15 minutes on, 15 minutes off while awake. This helps reduce swelling and numb pain.

Medications: Take all prescribed medications as directed by Dr. Olaes. If you're given antibiotics, complete the entire course even if you feel better. For over-the-counter pain relief, ibuprofen (Advil, Motrin) is often most effective for dental procedures as it reduces both pain and inflammation. Acetaminophen (Tylenol) can be alternated with ibuprofen if needed.

After 48 hours: Switch from ice to warm, moist heat applied to the outside of your face to help reduce any remaining swelling.

Eating and Drinking

Your diet plays a crucial role in protecting the graft site and promoting healing:

First 24-48 hours:

  • Stick to soft, cool foods like smoothies, yogurt, pudding, and ice cream
  • Drink plenty of water, but avoid using straws
  • Avoid hot beverages and foods
  • No alcohol while taking prescription medications

Days 3-7:

  • Continue with soft foods: scrambled eggs, mashed potatoes, pasta, soup (lukewarm)
  • Chew on the opposite side of your mouth
  • Gradually introduce slightly firmer foods as comfort allows

Avoid for 2 weeks:

  • Hard, crunchy foods (chips, nuts, popcorn, crackers)
  • Sticky foods (gum, caramel, taffy)
  • Spicy or acidic foods that might irritate the site
  • Small seeds or grains that could get lodged in the surgical area

Return to normal diet: Most patients can resume their regular diet after 2 weeks, but listen to your body and progress gradually.

Oral Hygiene

Keeping your mouth clean while protecting the graft site is essential:

First 24 hours: Do not brush, rinse, or spit. This allows the initial blood clot to form and protects the graft.

Days 2-14:

  • Gently brush your teeth, avoiding the surgical area
  • Use a soft-bristled toothbrush
  • Starting day 2, rinse very gently with warm salt water (1/2 teaspoon salt in 8 oz warm water) 2-3 times daily
  • If prescribed a special mouth rinse, use as directed
  • Do not use commercial mouthwash unless approved by Dr. Olaes

After 2 weeks: You can gradually return to your normal oral hygiene routine, but continue being gentle around the graft site until fully healed.

Activity Restrictions

Protecting your graft from disruption is crucial for successful healing:

First 48-72 hours:

  • Rest and limit physical activity
  • No bending over, lifting heavy objects (over 10 lbs), or strenuous exercise
  • Avoid activities that might increase blood pressure and cause bleeding

First week:

  • Light activities like walking are fine
  • Avoid exercise that makes you breathe heavily through your mouth
  • No swimming or hot tubs (risk of infection)

After 1 week: You can gradually return to normal activities as comfort allows, but avoid contact sports or activities that risk facial injury until the graft is fully integrated.

Sleep considerations: Keep your head elevated for the first few nights to minimize swelling.

Warning Signs - When to Call Us

  • Numbness or tingling that persists beyond the expected timeframe
  • Allergic reactions to medications (rash, difficulty breathing, severe nausea)
  • Feeling like the graft material is loose or coming out
  • Persistent nausea or vomiting
  • Difficulty opening your mouth after the first few days

Remember, it's always better to call with concerns than to wait and risk complications.

Recovery Timeline

Days 1-3: Peak discomfort and swelling. Focus on rest, ice, and following all restrictions.

Days 4-7: Swelling begins to subside. You may start feeling more comfortable and can gradually increase soft food options.

Week 2: Most acute symptoms resolve. You'll likely have a follow-up appointment to check healing progress.

Weeks 3-4: Continued improvement. Soft tissue should be well-healed, but bone integration is ongoing.

Months 1-3: Bone graft continues to integrate with your natural bone. Regular check-ups monitor progress.

Months 3-6: Full integration typically complete. Ready for next phase of treatment (such as implant placement).
